ISAR/SEISAR: a two step intervention
ISAR (Identification of Seniors At Risk) is a 6-item screening tool for seniors in the emergency department. The tool quickly identifies seniors who are at increased risk of a variety of adverse outcomes (including functional decline, death, institutionalization, return visits, high rates of hospitalization and use of other services). It was developed as a tool to be used as the first step of a 2-step intervention. “Seniors At Risk” detected with the ISAR tool are then evaluated using the SEISAR.
SEISAR (Systematic Evaluation and Intervention for Seniors At Risk) is a short, standardized, comprehensive tool for the evaluation of active geriatric problems in seniors in the emergency department. The SEISAR tool is an action-oriented checklist that incorporates recommendations for the most appropriate interventions. A semi-standardized questionnaire corresponding to each problem on the SEISAR tool, is described in the Clinical & Administrative Manual (available as mentioned below). Overall, SEISAR is a brief standardised comprehensive geriatric assessment, adapted to ED practice. It was developed as the second of the 2-step intervention, for “Seniors At Risk”, identified by the ISAR.
